Remove the Ref classes, as they won't be required
anymore once Managed and Managed::Data are separated.

The method is now optional, and we can simply avoid
calling it if all members an object has are themselves
garbage collected.

Module includes are discouraged because they may increase compilation
time. While debugging a broken core module include header, a number
of module include infractions were discovered in qtdeclarative. Replace
them with their more specific counterparts.

Keep the basic methods in ManagedVTable, but have
the Object related stuff in an ObjectVTable class.
