backport of upstream commit 603fd64dfa45 ("powerpc/book3s64/memhotplug:
enable memmap on memory for radix")

This patch is a backport of the following upstream commit:
commit 016fec91013cfb27c9f5a101a87ae8266537fed1

mm: move is_ioremap_addr() into new header file

Now is_ioremap_addr() is only used in kernel/iomem.c and gonna be used in
mm/ioremap.c. Move it into its own new header file linux/ioremap.h.
