    Found with W=2, remove unused macros in C files. In one case
    move the macro under the corresponding ifdef.

Johannes Berg fd1c3318f4 iwlwifi: mvm: validate notification size when waiting
When waiting for a notification and then processing it,
we also need to check the size of the data before we use
it. Most places do that already, but fix the remaining
ones to do it as well.

Luca Coelho 754f890a3a iwlwifi: remove all occurrences of the FSF address paragraph
The Free Software Foundation address is superfluous and causes
checkpatch to issue a warning when present.  Remove all paragraphs
with FSF's address to prevent that.

Sara Sharon 176aa60bf1 iwlwifi: mvm: set HCMD_NAME for PHY_DB as well
Currently it is logged as UNKNOWN.
Also, 0x6c seems to be the permanent ID for this command, remove
incorrect comment and uncomment the command from the commands
list.

Sara Sharon 0ec84d1d1e iwlwifi: mvm: make phy_db size dynamic
Driver is agnostic to the number of the phy_db entries and
only serves the firmware as a pipe to move the data from init
image to RT image.
As the size of the arrays may change (as it does in 9000 device)
allocate it dynamically. Firmware sends the largest index first
so we can use this to know how much we should allocate.
